Scoring Services:
Scoring Services are available to all faculty and staff at the University. Scanning is done on a Scantron 4000 Optical Mark Reader. The scanner is capable of scanning pencil responses only.
Location and availability of scoring services:
Scoring Services is located in 24 Capen Hall, Instructional Technology Services.
Business hours are from 8:30 A.M. to 5:00 P.M. Monday thru Friday.

REQUIREMENTS
To get exams scored, you must submit
- A completed Request for Optical Mark Scanning Form
- Key Sheet(s) for each exam or versions of an exam
- Student Answer Sheets
The Request for Optical Mark Scanning form is available in 24 Capen Hall. This is a scannable form; it cannot be duplicated or faxed. Please use a number 2 pencil to complete this form. They can also be mailed to you by calling 645-3537.ANSWER SHEETS
Answer sheets must now be purchased directly from the vendor. You have two choices. The majority of the departments use the NCS P099B form which is green. Your other option is the Scantron Student Enrollment/Score Sheet (F-1712-ERI) this form is red.
The National Computer Systems forms can be purchased directly from the company by contacting 1-800-367-6627.
The Scantron Forms can be ordered by logging on www.Scantron.com
THESE ANSWER SHEETS CANNOT BE MIXED. EXAM LENGTH IS LIMITED TO 200 QUESTIONS WITH 5 OPTIONS.
REQUIREMENT FOR STUDENT ANSWER SHEET
Students must use a Number 2 pencil only.Each student answer sheet Must include:
- The student's 8 digit person number
- The student's name
